The headline and lede are drafts. Confirm the tagline (current site uses “Southern California’s Trusted Hospice Provider”; “Trusted” is the category’s most overused word).
“24/7 clinical line,” the office locations, and the CMS-aligned framing are stated as fact; quick to confirm at kickoff, but unconfirmed here.

Confirmed by Alan (2026-09-01 email): Medicare certified, California licensed, CHAP certified, HMDCB (Hospice Medical Director Certification Board), and CHAPCA member (California Hospice and Palliative Care Association). Still to add for official display: the official marks/logos and the CDPH license number. Current seals are typographic, not official agency logos.
The map is an illustrative branded graphic from the three known offices (LA, Riverside, Apple Valley). A hospice service area is CMS/CDPH-defined; confirm the exact licensed coverage, full office addresses, and whether more locations belong on it.
The bottom form is a non-PHI general-contact form by design, with a visible notice not to enter patient health information. It delivers into a BAA-covered Microsoft 365 mailbox via Microsoft Graph, with no third-party email service in the path. Clinical referrals (which carry PHI) are taken by phone, fax, or EMR, never through this site. Turnstile and rate limiting are added before the form goes public.
